COMPLETE CARE
Becoming a member of Complete Care covers you, your spouse and legally dependent children for any additional out of pocket expense within a calendar year. Complete Care is not insurance; it is a membership agreement.
Creek County Ambulance is staffed 24 hours a day with highly trained emergency personnel. Should an emergency strike you or your loved ones, count on us to provide emergency medical care. The Complete Care program offers freedom from financial worries regarding emergency ambulance transportation within our service area of Creek County.
The ambulance station headquarters are located at 123 E. Hobson Ave., Sapulpa Ok. We also have sub stations in Bristow and Drumright.
Ambulance transportation must be medically necessary. Trips to Doctor’s office’s or transport after being discharged from a hospital are not considered a medical necessity for ambulance transportation.
If after reading this, you or someone you know desire to enroll in the Complete Care program give our office a call at 918-227-4111. An enrollment form will gladly be sent to you in the mail or you may drop by our headquarters to fill one out. The cost for the membership agreement is $ 85.00 per year.
This membership is not transferable or refundable.
Your annual renewal each year will serve as your agreement for us to file necessary claims with your insurance company.
